PHYSICAL FORMS – PIAA Section 5 & Section 6
**Section 5 Health History Form – To be completed by the parent/guardian in its entirety and presented to the Authorized Medical Examiner performing the physical for review.
**Section 6 Preparticipation Physical Evaluation – To be filled out by the Authorized Medical Examiner and returned to the athletic office prior to the first practice or contest as part of the registration process.
Section 8 ReCertification by Licensed Physician
**This is NOT the physical form. It is a clearance form.**
Have your treating doctor fill out this form if your student-athlete was injured and is now being cleared for participation.
Home School Eligibility Form
All homeschool students participating in Butler Athletics must fill out and turn in this form every Friday by 2:30pm during the sports season.
Cyber Charter School Eligibility Form
All cyber school students participaing in Butler Athletics must turn in this form every Friday by 2:30pm during the sports season.
